not provided Uncertain:2
Not observed at significant frequency in large population cohorts (gnomAD); In silico analysis supports that this missense variant has a deleterious effect on protein structure/function; Reported in an individual with sporadic juvenile polyposis (Calva-Cerqueira et al., 2009); Published functional studies demonstrate protein expression levels similar to wild-type and increased BMP signaling and intracellular localization, which was not statistically significant when compared to wild-type (Howe et al., 2013); This variant is associated with the following publications: (PMID: 18823382, 23433720, 33032550, 31769494) -

Juvenile polyposis syndrome Uncertain:2
This sequence change replaces arginine, which is basic and polar, with histidine, which is basic and polar, at codon 254 of the BMPR1A protein (p.Arg254His). This variant is present in population databases (rs766908700, gnomAD 0.01%). This missense change has been observed in individual(s) with sporadic juvenile polyposis (PMID: 18823382). ClinVar contains an entry for this variant (Variation ID: 411647). Invitae Evidence Modeling incorporating data from in vitro experimental studies (internal data) indicates that this missense variant is not expected to disrupt BMPR1A function with a negative predictive value of 95%. Experimental studies have shown that this missense change does not substantially affect BMPR1A function (PMID: 23433720, 31769494). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
This missense variant replaces arginine with histidine at codon 254 of the BMPR1A protein. Computational prediction suggests that this variant may have deleterious impact on protein structure and function (internally defined REVEL score threshold >= 0.7, PMID: 27666373). A functional study indicated that the variant protein showed reduced localization to the cellular membrane however protein levels and BMP signaling were similar to or greater than wild type (PMID: 23433720). This variant has been reported in individuals affected with juvenile polyposis (PMID 18823382) and primary ovarian insufficiency (PMID 28505269). This variant has been identified in 4/251344 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -

This alteration is classified as benign based on a combination of the following: seen in unaffected individuals, population frequency, intact protein function, lack of segregation with disease, co-occurrence, RNA analysis, in silico models, amino acid conservation, lack of disease association in case-control studies, and/or the mechanism of disease or impacted region is inconsistent with a known cause of pathogenicity. -
