Variant summary: FLCN c.1523A>G (p.Lys508Arg) results in a conservative amino acid change in the encoded protein sequence. Four of five in-silico tools predict a damaging effect of the variant on protein function. The variant allele was found at a frequency of 0.00019 in 251484 control chromosomes (gnomAD), predominantly at a frequency of 0.00042 within the Non-Finnish European subpopulation in the gnomAD database. The observed variant frequency within Non-Finnish European control individuals in the gnomAD database is approximately 336 fold of the estimated maximal expected allele frequency for a pathogenic variant in FLCN causing Birt-Hogg-Dube Syndrome phenotype (1.3e-06), strongly suggesting that the variant is a benign polymorphism found primarily in populations of Non-Finnish European origin. Experimental evidence evaluating the impact of the variant on protein function found that the variant is able to suppress tumor growth to levels comparable to wild-type (Nahorski_2011), but the variant was also not able to completely rescue normal kidney development in a FLCN deficient mouse model (Hasumi_2017). The following publications have been ascertained in the context of this evaluation (PMID: 28007907, 23784378, 21538689). Six ClinVar submitters have assessed the variant since 2014: two classified the variant as uncertain significance, two as likely benign, and two as benign. Based on the evidence outlined above, the variant was classified as likely benign. -
