Variant classified as Uncertain Significance - Favor Pathogenic. The Thr1479Ile variant in MYH7 has been identified by our laboratory in two individuals with HC M and was not identified in large population studies. Threonine (Thr) at positio n 1491 is highly conserved in mammals and across evolutionarily distant species and the change to isoleucine (Ile) was predicted to be pathogenic using a comput ational tool, which was validated by our laboratory using a set of cardiomyopath y variants with well-established clinical significance. This tool's pathogenic p rediction is estimated to be correct 94% of the time (Jordan 2011). In addition , missense variants in MYH7 are often associated with HCM. In summary, although the data support that the Thr1479Ile variant may be pathogenic, additional stud ies are needed to fully assess its clinical significance. -

Variant of Uncertain Significance due to insufficient evidence: This missense variant replaces threonine with isoleucine at codon 1479 of the MYH7 protein. Computational prediction tools and conservation analyses suggest that this variant may have deleterious impact on the protein function. Computational splicing tools suggest that this variant may not impact RNA splicing. To our knowledge, functional assays have not been performed for this variant nor has this variant been reported in individuals affected with cardiovascular disorders in the literature. This variant is rare in the general population and has been identified in 0/277264 chromosomes by the Genome Aggregation Database (gnomAD). Available evidence is insufficient to determine the role of this variant in disease conclusively. -

Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is expected to disrupt MYH7 protein function with a positive predictive value of 95%.
