Variant summary
Our verdict is Likely benign. The variant received -4 ACMG points: 0P and 4B. BS2
The NM_002880.4(RAF1):c.1355C>T(p.Thr452Met) variant causes a missense change involving the alteration of a conserved nucleotide. The variant allele was found at a frequency of 0.0000167 in 1,613,458 control chromosomes in the GnomAD database, with no homozygous occurrence. Variant has been reported in ClinVar as Uncertain significance (★★). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. T452K) has been classified as Uncertain significance.

The T452M variant has not been published as a pathogenic variant, nor has it been reported as a benign variant to our knowledge. The T452M variant is a non-conservative amino acid substitution, which is likely to impact secondary protein structure as these residues differ in polarity, charge, size and/or other properties. This substitution occurs at a position that is conserved across species and in silico analysis predicts this variant is probably damaging to the protein structure/function. However, only one missense variant in a nearby residue (I448V) has been reported in the Human Gene Mutation Database in association with acute myeloid leukemia (Stenson et al., 2014), indicating that this region of the gene is not known to harbor many disease-causing variants. Furthermore, the 1000 Genomes Project reports T452M was observed in 3/978 (0.3%) alleles from individuals of South Asian ancestry, indicating it may be a rare (benign) variant in this population. Therefore, based on the currently available information, it is unclear whether this variant is pathogenic or rare benign. -
Cardiovascular phenotype Uncertain:1
The p.T452M variant (also known as c.1355C>T), located in coding exon 11 of the RAF1 gene, results from a C to T substitution at nucleotide position 1355. The threonine at codon 452 is replaced by methionine, an amino acid with similar properties. This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Based on the available evidence, the clinical significance of this variant remains unclear. -
RASopathy Uncertain:1
This sequence change replaces threonine, which is neutral and polar, with methionine, which is neutral and non-polar, at codon 452 of the RAF1 protein (p.Thr452Met). This variant is present in population databases (rs555781462, gnomAD 0.01%). This variant has not been reported in the literature in individuals affected with RAF1-related conditions. ClinVar contains an entry for this variant (Variation ID: 372553). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is expected to disrupt RAF1 protein function with a positive predictive value of 95%. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
