The p.R486Q variant (also known as c.1457G>A), located in coding exon 10 of the BMPR1A gene, results from a G to A substitution at nucleotide position 1457. The arginine at codon 486 is replaced by glutamine, an amino acid with highly similar properties. This amino acid position is highly conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -

Computational prediction suggests that this variant may not impact protein structure and function (internally defined REVEL score threshold <= 0.5, PMID: 27666373). Three of five in-silico tools predict a benign effect of the variant on protein function. The variant allele was found at a frequency of 8e-06 in 251492 control chromosomes. The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.1457G>A has been reported in the literature in at least one individual with a personal history of breast cancer and a family history of breast and other cancers, but was classified by the authors as a VUS (Bhai_2021). This report does not provide unequivocal conclusions about association of the variant with Juvenile Polyposis Syndrome. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported.
